About the role:
We're seeking an experienced Registered Nurse to join a new and growing team as a Clinical Coordinator. In this varied and rewarding role, you'll build long-term relationships with clients and utilise your clinical and training skills within a supportive national network, working from a modern shared office with strong technology and collaborative spaces. You'll combine your clinical expertise with problem-solving skills to coordinate high-quality, person-centred rehabilitation and disability support services for clients with complex needs.
What you'll be doing:
- Meeting clients and their families to create and review individual support plans.
- Conducting client assessments and monitoring changes in health status.
- Coordinating homecare packages for clients with disabilities and/or serious injuries.
- Training, mentoring and assessing support workers to ensure safe, high-quality service delivery.
- Liaising with funders (ACC, NASC, DHBs) and other health providers to ensure seamless support.
- Ensuring compliance with clinical standards, quality systems, and contract KPIs.
What you'll bring:
- Must hold current New Zealand Annual Practicing Certificate (APC)
- Minimum of 3 years' nursing experience in New Zealand.
- Experience working in community health, complex disability and rehabilitation preferred.
- Current clinical competencies in the management of PEG, stoma, SPC's, bowel and bladder support etc. preferred but not essential.
- Strong communication and relationship-building skills.
- Ability to work autonomously and collaboratively.
- Full NZ driver's licence and reliable vehicle.
